Physical Activity Referral Programmes

Referral Criteria: 

Referred adults for all Physical Activity sessions MUST be:

  • 18 years or above
  • Give their consent for referral to Cornwall Healthy Weight
  • Be motivated and ready for change

Exclusion Criteria:

Unfortunately we are unable to accept referrals for adults who have any of the exclusions listed below: 

  • Adults with a BMI >40 OR >35 with comorbidities
  • Unstable angina
  • Resting BP - Systolic BP >180 mmHg and/or Diastolic BP >110 mmHg
  • Resting tachycardia >100bpm
  • Uncontrolled atrial or ventricular arrhythmias
  • Recent complicated Myocardial Infarction and/or awaiting further investigation
  • Unstable or acute heart failure
  • Uncontrolled diabetes
  • Acute systemic disease such as cancers
  • Neuromuscular, musculoskeletal or rheumatoid disorders that are exacerbated by exercise
  • Unmanaged pain
  • Clients with an unstable psychiatric disorder

Physical activity reviews

These one to one sessions aim to increase an individual’s physical activity level in their local community, adapting exercise based on exercise referral guidelines. The Physical Activity Reviews are also a core role within the RCHT bariatric / weight management service.

All sessions are arranged at times to suit both the referred individual and the professional supporting them.

Content:

  • Review of the individual’s medical history following exercise referral guidelines
  • The importance of being active and benefits to health
  • Understand factors that affect an individual’s participation in activity
  • Individuals encouraged to increase their level of physical activity
  • Discussion of local support, community groups and classes
  • Provision of useful resources, tools and action plans
  • Individualised support and guidance to benefit their health
  • Empower and encourage positive behaviour change

  • Referrals accepted from primary care and all healthcare professionals

Swimming for health 

These sessions support individuals, including non-swimmers, who may not have the confidence to attend public pool swimming sessions and would benefit from increasing their activity.

Twelve sessions are offered to an individual over a 16 week period. Sessions are closed to the public and are offered in various locations throughout Cornwall.

Content:

  • Support individual to increase confidence in the water
  • Learn rehabilitation exercises or improve swimming technique
  • Provision of pool hoist and additional swimming aids
  • Individualised support and guidance to benefit their health
  • Empower and encourage positive behaviour change

Referrals:

  • Referrals accepted from primary care and all healthcare professionals
  • Self referrals accepted from eligible individuals across Cornwall

Walking for health

This is a national support programme run by the Ramblers and Macmillan Cancer Support. By sharing their walking and health expertise it enables Cornwall Healthy Weight to support volunteer led health walks across Cornwall to help more people, including those living with medical conditions, discover the joys and health benefits of walking locally.

Content:

  • Start walking in a safe and friendly environment
  • Get to know the local area and meet new people
  • Increase energy levels and generally improve health
  • Health walks are led by trained volunteer walk leaders

Referrals:

  • Referrals accepted from primary care and all health and social care professionals
  • Self referrals accepted from eligible individuals across Cornwall
