The Acute Liaison Team for Adults and Children with Learning Disabilities and / Autism Spectrum Conditions is based at Treliske hospital.
They can support you with your plans to go to hospital and during your time in hospital.

The screening liaison nurses support people with learning disabilities and/or autistic people to understand why screening is important and what to expect at a screening appointment.
The CHAMPs team teamed up with our amazing Screening Liaison nurses to create a fun and catchy song all about the importance of screening because they wanted to make sure people with a learning disability and/or autistic people understand how important it is to get checked regularly to stay on top of your health:
They are useful to tell staff more about you.
They also tell staff about your needs and likes.
